Community is defined as health-care positions within Primary Care, Public Health, Home Health, Mental Health and substance Use and Personalized Support and Stabilization. Opportunities in these areas exist for both staff and medical staff interested in providing care outside of an acute environment. Working in the Community allows health-care providers the ability to provide health services to clients in homes, clinics, drop-in centres, community sites, and child care facilities.

Community Health Centres
At our Community Health Centres, VCH teams focus specifically on the top five per cent of Vancouver’s most complex clients who are often marginalized and have multiple health (clinical and psychosocial) conditions that impact their day-to-day living (i.e., functionality). Our clients frequently experience homelessness, mental health and substance use disorders, live in poverty, and have difficulty accessing traditional health-care services in a doctor’s office-type setting.
Home Support
Our philosophy is that home is best, as most people wish to remain in their own homes for as long as possible. We are committed to the quality of life by supporting independence, health, and wellness by delivering person- and family-centered care. Home support services can be provided on a short-term basis, such as after leaving the hospital or as part of end-of-life care, or over a longer period, such as several months or years. We have opportunities for you to join our team as a Community Health Worker II (Health Care Assistant/Care Aide). Be a core part of our communities as caregivers and make a significant impact on the quality of life of their clients and patients.
Mental Health and Substance Use (MHSU)
We offer mental health services in many of our locations. Opportunities for mental health professionals can be found across the spectrum, from community care to high acuity. Special programs include art therapy, early psychosis intervention, mood disorders, concurrent disorders and peer support. Through our multi-disciplinary care teams and partnerships with external agencies, we provide diagnosis, treatment, individual and group therapy, rehabilitation, consultation, emergency and urgent services, and long-term care services.
Urgent and Primary Care Centres
Urgent and Primary Care Centres (UPCC) will provide care for people with unexpected, non-life-threatening conditions who need medical attention within 12-24 hours but who don’t require a visit to the emergency department – this includes sprains, cuts and wounds, an infection, mild asthma attack and less serious childhood illness and injury. UPCCs won’t replace the emergency department at the hospital for life-threatening conditions (such as heart attack, stroke, major trauma, acute psychiatric care, etc.). The multidisciplinary health-care team will be staffed by general practitioners, nurse practitioners, registered nurses and social workers.
UPCCs are intended to be an additional service in the community, working in concert with all touchpoints for care to provide appropriate urgent services to patients when and where they need them.
Primary Care Networks
Primary Care Networks (PCN) organize different teams of health-care providers together within a geographical community – this includes nurses, pharmacists, licensed practical nurses, registered nurses, mental health and addictions specialists, social workers, pharmacists, dietitians and/or other health-care professionals. These networks will be made up of participating family physicians and primary care offices (also known as Patient Medical Homes or ”PMH”), Primary Care Community Health Centres, Urgent and Primary Care centres, and services delivered by not-for-profit community health partners.
Adopting a team-based care approach in PCNs aims to increase clients’ access to primary care services and provide strong, effective links to hospitals and diagnostic service providers.
Public Health
VCH Public Health works to improve the health and well-being of residents by preventing disease, promoting health, and prolonging life among the population as a whole. Staff at the “field level” are assigned to geographic districts and often carry a broad mandate for the delivery of health protection programs in those areas. Hours of work vary across health service areas, with the availability of compressed work-week options in some areas.
