The ALS and Related Disorders (ALSRD) Clinic provides ongoing assessment, intervention, consultation and educational services to people with ALS and related motor neuron diseases (MNDs).

How to access

  • Check eligibility

    We encourage referral to the ALS and Related Disorders Clinic for all patients where the referring doctor believes ALS/PLS to be highly likely. Patients do not need to have a definite diagnosis of ALS or PLS before referral.

  • Get a referral

    Referrals must be made by a physician or nurse practitioner. Have your physician or nurse practitioner complete the Amyotrophic Lateral Sclerosis (ALS) and Related Disorders Program referral form and fax the completed form to (604) 822-2611.
