Harm reduction: Toward the Heart 

Support services for adults aged 19 or older, who require support to access mental health and substance use services.  Intended for people requiring intensive support to establish and maintain connection to social, health and mental health services, community connection and to improve housing stability.  This service is harm reduction based, support people in meeting self defined goals, and aims to stabilize individuals and  transition them to less intensive supports. 

How to access

Service is voluntary. 

  • Adults aged 19 and older living in the Sunshine Coast area. 
  • Individuals can self-refer by calling 604-885-8678.
