The Older Adult Mental Health Team offers services to help improve the quality of life of older adults (and their caregivers), who are vulnerable due to a combination of psychiatric, medical and social factors.
Our services
Older Adult Mental Health Team North Shore is an interdisciplinary team providing community-based mental health services at no cost.
We offer psychiatric assessment and treatment for older adults who may require outreach services. Our approach is to match services with individual needs. We provide case management and psychiatric services as needed and may also include liaising with pharmacies, families, and other community supports. Services include:
- Interdisciplinary assessment, treatment, and consultation
- Education to clients, their care partners, and community members
Specialized services also include:
- Geri-psychiatric assessment and consultation
- Case management
- Psychosocial rehabilitation
- Functional assessment
- Group engagement
How to access

Check eligibility
We support individuals, generally aged 65+ living on the North Shore who require an interdisciplinary team and
meet one or more of the following criteria:- A progressive cognitive disorder with associated psychiatric and/or behaviour management problems and/or co-occurring misuse of
alcohol, prescription medications, or other substances - A recently developed serious mental illness and physical and functional decline
- A severe and persistent mental illness with cognitive, physical, or functional decline
Individuals under the age of 65 will be seen if they have a progressive memory disorder with associated psychiatric and/or behaviour

There is no cost for services.
Clients remain under the care of their General Practitioner or Nurse Practitioner.

- Monday: 8:30 a.m. to 4:30 p.m.
- Tuesday: 8:30 a.m. to 4:30 p.m.
- Wednesday: 8:30 a.m. to 4:30 p.m.
- Thursday: 8:30 a.m. to 4:30 p.m.
- Friday: 8:30 a.m. to 4:30 p.m.
- Saturday: Closed
- Sunday: Closed
