The Overdose Outreach Team connects people who have recently experienced an opioid overdose and/or who are at high risk for opioid overdose to support.

What this service includes

This team assists with navigation to appropriate services, supports access to substance use stabilization and treatment services, supports with engagement and retention in services. Offers harm-reduction supplies including naloxone and overdose prevention education.

Service Includes:

  • Navigation to appropriate services (mental health and substance use programs such as counselling, ACCM, etc.)
  • Support to access Opioid Agonist Therapy (OAT) (e.g., methadone, suboxone etc.)
  • Complex care coordination and planning for clients with barriers to engagement and retention in services
  • Provision and access to harm-reduction supplies
  • Overdose prevention education
  • Treatment application support
