While in hospital, many individuals work together as a team to help the patient with their recovery and discharge plan.

Get to know the different stroke team members

  • Physicians and Nurse Practitioners

    Patients admitted under the neurology team are cared for by both our stroke neurology physicians and stroke nurse practitioners. 

    Stroke Neurology Physicians

    • Specialise in diagnosis, treating and managing stroke patients
    • Deciding on medications and procedures in the acute stage of a stroke
    • Focus on preventing, identifying and treating various types of strokes to minimize brain damage and improve recovery outcomes

    Stroke Nurse Practitioners

    • Assess, diagnose and manage stroke-related conditions, prescribe treatment and support recovery while collaborating with the medical team and other healthcare professionals. 

    Stroke Hospitalist

    • Physician who specializes in the care of stroke patients during their hospital stay. They focus on managing stroke treatment, monitoring recovery, and coordinating care with other specialists to ensure optimal patient outcomes
  • Nurses (RNs or LPNs)

    • Provide care and support to the patient
    • Speak up for the patients when they are unable
  • Clinical Pharmacists

    • Can help patient and families understand complicated medication routines
    • Monitor dosing and check interactions of all medications
  • Physiotherapists (PT)

    • Test and identify physical problems
    • Set up a treatment plan for your physical recovery and independence
    • If appropriate, teach family members how to assist with recovery
  • Occupational Therapists (OT)

    • Work with the patient to perform daily activities such as feeding, washing and dressing, cooking and banking
    • Help the patient relearn or adapt the way of doing activities of daily living
  • Speech Language Pathologists (SLP)

    • Assess patient’s speaking, understanding, reading and writing
    • Recommend strategies
    • Arrange follow-up services if appropriate
    • For some patients, the SLP may assess the patient’s swallow and recommend exercises to improve their swallowing abilities
  • Registered Dieticians (RD)

    • Help patient receive the appropriate nutritional care
    • Can answer questions about food and nutrition
  • Social Workers (SW)

    • Can provide information and support in relation to government benefits to help subsize financial care needs
    • Can provide emotional and legal supports
  • Care Management Leaders (CML) and Transitions Services Team (TST)

    • Work together with the patient, family and allied health team to organize and plan an appropriate and safe discharge plan home
    • TSTs are available to help arrange home support services from the patient’s local health unit
