How to acess
Patients are typically referred to this service by a doctor. Concerned individuals, family members, friends or other health care professionals may also contact us. However, for an assessment to be scheduled, a referral must be provided by a doctor.
Download the Alzheimer referral package and fax the completed form to our clinic at 604-297-9684.
There is a waiting list for an assessment. An appointment is made once all previous medical reports are obtained. A close family member or friend is asked to accompany the patient to appointments as a support person. This is important because they may be able to provide historical information and also to give the patient support throughout the assessment.
Preparing for your appointment
The clinic is located on the second floor of the the Djavad Mowafaghian Centre for Brain Health on the UBC Hospital campus. The clinic is open from 8:00 a.m. to 4:00 p.m. Monday to Friday. Please check in at our reception desk a few minutes before your appointment.
