Referral is required. Have your care provider completed the Rapid Access Spinal Clinic referral form and fax the completed form to 604-984-3748.
How to get here
- This service is located in the new Paul Myers Tower on the main floor by the 14th street building entrance in the Outpatient Care Centre at Lions Gate Hospital.
- Parking is available in the hospital parking lot located on the 15th street.
