Kidney clinics provide education, support and counseling to patients who have Chronic Kidney Disease (CKD) and their families.
Services include individual and group self-management classes, nutritional counselling and personal lifestyle counseling. The interdisciplinary team includes:
- nurses,
- social workers,
- dietitian and
- pharmacist.
The goals of the team in cooperation with kidney specialist are to:
- Help patients delay the progression of kidney disease.
- Educate patients and their families about treatment choices for chronic kidney disease (peritoneal dialysis, hemodialysis, home hemodialysis and kidney transplantation or end of life care).
- Encourage a healthy lifestyle.
- Provide support to patients and their families.
