The Leon Judah Blackmore Centre for Sleep Disorders at UBC Hospital was established in 1981 and is the provincial referral centre for complex sleep disorders. Our mission is to improve health and quality of life by improving sleep. We are a multi-disciplinary specialist program including specialists in psychiatry, respirology, neurology, and internal medicine.
CPAP Education services are also provided. A broad range of sleep-related diagnostic services are provided including attended inpatient polysomnography, home sleep studies, actigraphy, and multiple sleep latency testing. The Centre is also active in teaching and research related to sleep disorders.
Patient referrals
Referrals can only be made by a health care professional.
All referrals must be made by a physician, nurse practitioner, or specialist with a valid MSP number. If you are a patient, talk with your primary care provider for a referral to the program, as patients cannot self-refer themselves to the Centre.

Sleep Medicine consult referral
Eligibility criteria
- Our clinic serves patients aged 16 and over only. For patients under age 16, please refer to the BC Children’s Hospital Sleep Clinic
- We accept referrals from all of BC and the Yukon for patients with sleep disorders.
Referral instructions
For a consultation with a sleep medicine physician, a referral by a physician, nurse practitioner, or specialist with a valid MSP number is required. The referrer needs to complete a Sleep Disorder Consultation Referral Request (Form B) referral form and fax to 604-822-9744.
All consultation requests are triaged by a physician to identify patients requiring urgent assistance, and to ensure patients are assigned to the appropriate sleep specialist (respirology, psychiatry, neurology, internal medicine).

Home Sleep Apnea Test (HSAT) referral
Eligibility Criteria
HSAT (without a sleep medicine consultation) can be ordered by a healthcare provider with a valid MSP number. HSAT can usually be done within a few weeks.
Please ensure the patient meets the eligibility criteria as outlined in Form A: Requisition for Home Sleep Apnea Test. The patient needs to be able to come to UBC Hospital to pick up and return the HSAT, and follow-up of HSAT results will be through the referring provider.
Referral Instructions
For patients requiring a home sleep apnea test only (without a sleep medicine consult), complete a Requisition for Home Sleep Apnea Test (HSAT) and Referral Request (Without Sleep Disorder Consultation).
